The Investigation Quality-Corrective Action Link: Why Your Fix Failed Before You Implemented It

When a corrective action fails to prevent recurrence, the first questions usually focus on implementation. Was the action completed properly? Was there enough ownership? Did supervisors reinforce the change? Was the new process embedded into normal work? Those are reasonable questions, but they start too late. In many cases, the corrective action was already compromised before anyone began implementing it. The problem occurred during the investigation, when the organisation gathered incomplete evi

Who Was Responsible for the Death Star Incident?

Let’s investigate one of the most significant asset-loss events in galactic history. A moon-sized battle station is destroyed during hostile operations. There is catastrophic equipment loss, extensive loss of life, major disruption to operations and, presumably, a fairly uncomfortable meeting scheduled with the Emperor the following morning. The immediate cause appears obvious. Luke Skywalker fired two proton torpedoes into a thermal exhaust port. Case closed. Corrective action: locate Luke Skywalker

Your Investigation Data Can Only Tell You What You Thought to Look For

What new research on AI tells us about human thinking, investigation quality and ICAMArtificial intelligence can process an extraordinary amount of safety data. It can classify reports, identify patterns and help organisations make sense of information that would take people months to review manually. But there is a catch. AI can only analyse what people recorded in the first place. If an investigation captures the immediate event but misses the conditions that shaped it, those conditions effectiv

Multi-Jurisdictional ICAM Investigations: Conducting an Investigation Across Regulatory Regimes, States and Countries

A serious incident does not always respect organisational or jurisdictional boundaries. A vehicle may be registered in one state, operated by an employee based in another and involved in an incident while working at a third-party site elsewhere. A piece of equipment may be designed in Europe, manufactured in Asia, maintained by an Australian contractor and fail during operations in Africa. A logistics incident may involve a transport operator, port authority, principal contractor and equipment su

The Capability Gap Between the Training Room and the Next Incident

There is a significant difference between completing investigation training and being ready to lead an investigation when a serious incident occurs. Training can provide the methodology, tools and opportunity to practice. It can give an investigator a structured process to follow, help them understand PEEPO, develop their interviewing technique and teach them how to move from evidence through analysis to findings and recommendations. In the training room, there is also something else available th

Can AI Plan Your Investigation?

AI can produce an investigation plan remarkably quickly. Give it a short incident description and it can suggest witnesses, documents, photographs, equipment records, procedures and other information that might be relevant. Ask it to organise those sources using PEEPO and, with the right instructions, it can produce something that looks very much like the beginning of an investigation plan. That capability is useful. It is also where investigators need to be careful. In this series, we have been e

Why Good Investigators Work Backwards

One of the defining features of ICAM analysis is the direction in which the investigation team works through the four contributing factor categories. The ICAM model presents Organisational Factors, Task or Environmental Conditions, Individual or Team Actions, and Absent or Failed Defences, but when undertaking the analysis, the investigator works in the opposite direction. We start on the right with the Absent or Failed Defences and work progressively to the left, through the Individual or Team

What Software Can and Cannot Surface From Your Investigations

Investigation software has changed what organisations can do with incident information. Instead of individual reports sitting in folders, shared drives or separate business systems, investigation data can increasingly be structured, searched, compared and analysed across multiple events. That creates opportunities that were difficult to achieve when every investigation effectively existed as a standalone document. Used well, technology can help an organisation see patterns across investigations,

When to Declare a Conflict of Interest in an Investigation

The credibility of an investigation depends on more than the technical quality of the evidence gathered or the analysis undertaken. It also depends on the integrity of the investigation process and the confidence stakeholders can have that the people conducting the investigation are able to approach the evidence objectively. For this reason, actual and perceived conflicts of interest need to be identified, declared and appropriately managed from the outset. A conf

Human Thinking in ICAM

AI is moving quickly into incident investigation. That can be useful. But faster processing does not automatically mean better investigation. AI can help search, organise, compare and summarise information. It can highlight gaps, generate questions and support drafting. What it cannot do is remove the need for investigator judgement. That is why we developed Human Thinking in ICAM. This practical guide looks at:- where AI can assist- where human judgement must remain- evidence versus assumption- work

Organisational Culture in ICAM: Look for the Evidence, Not the Label

A question came up recently during a live Q&A session in our ICAM Mastery program: How do you identify Organisational Culture when analysing the Organisational Factors in an ICAM investigation? It is a good question because Organisational Culture, or OC, can be one of the more difficult Organisational Factors for investigators to identify and, importantly, to substantiate. We can photograph a missing guard, examine a procedure, review a training record or establish that a risk assessment failed t

Better Investigations Will Not Come From More Automation

Why ICAM investigations remain a thinking discipline, and why investigator development matters more than ever In our previous article, The Limitations of AI in ICAM Investigations, we explored an increasingly important question for investigators and organisations: where can artificial intelligence genuinely assist an investigation, and where does reliance on it begin to undermine the investigative process? The conclusion was not that AI has no place in investigations. Used appropriately, technolog

ICAM vs Root Cause Analysis: Why Modern Incident Investigations Need More Than a Single Cause

“What was the root cause?”It is one of the most common questions asked after a workplace incident. The question sounds reasonable. Something went wrong, so the organisation wants to identify the cause, correct it and prevent the incident from happening again. The problem is not the intention behind the question. The problem is the assumption built into it. Asking for the root cause suggests that an incident can be traced back to one decisive failure. It encourages investigators to search for a sing

People deserve fairness, dignity and respect before they deserve judgement

Every workplace investigation begins with an event. It may be an injury, a near miss, a quality failure, an environmental incident, a complaint or, in the worst cases, a fatality. Almost immediately, the organisation begins looking for answers. Unfortunately, many organisations also begin looking for someone to blame. The language is often more subtle than that, but the thinking is familiar."Who made the mistake?""Who failed to follow the procedure?""Who

The Business Case for Investigation Training: Why the Cost of Prevention Is Almost Always Lower Than the Cost of Failure

Organisations rarely struggle to justify spending money after a serious incident. They approve legal support, engage consultants, replace damaged equipment, pay insurance excesses, compensate injured workers, manage regulators and invest heavily in corrective actions. What they often struggle to justify is investing a comparatively small amount beforehand to ensure their people know how to investigate incidents properly. This is one of the great paradoxes of modern business. Organisations routine

The Terminator vs Cam

Who Would Actually Be Better at an ICAM Investigation? Most workplace investigations fail for one reason:They start hunting for who instead of understanding why. That sounds obvious. But it still happens every day. An incident occurs. Pressure builds immediately. Operations want answers. Management wants accountability. The workforce wants certainty. And before long, the investigation becomes a search for: • the mistake • the operator • the breach • the “human error” • the person who caused it. In ot

The Psychological Safety Paradox: When Safety Starts Undermining Accountability

Has Psychological Safety Gone Too Far? Psychological safety has become one of the most influential workplace concepts of the last two decades. It appears in leadership programs, culture strategies, boardroom discussions, employee engagement surveys, and organisational transformation initiatives. It is regularly cited as a critical ingredient for innovation, collaboration, learning, and performance. Few management concepts have gained such widespread acceptance so quickly. That popularity is underst

John Wick vs ICAM

What Workplace Investigators Can Learn from the World’s Most Dangerous Problem Solver Introduction At first glance, John Wick and workplace incident investigators appear to have very little in common. One is a fictional assassin who resolves conflict with precision shooting, tactical movement, and an alarming body count. The other spends their time reviewing procedures, interviewing witnesses, building timelines, and analysing failed controls. But underneath the surface, there are surprisingly strong

What The Karate Kid Can Teach Us About ICAM Mastery There’s a reason The Karate Kid still resonates decades later. On the surface, it’s a story about martial arts, bullying, discipline, and resilience. But underneath? It’s actually a masterclass in how people learn to think. And if you’ve ever watched a new investigator stumble through their first workplace incident investigation, you’ll recognise Daniel La Russo immediately. Eager. Reactive. Focused on the obvious. Looking for quick answers. Meanwhile,

How ICAM Mastery Builds Credibility With Regulators, Workers, and Leadership

In modern workplaces, credibility is one of the most valuable assets a safety professional, investigator, or organisation can possess. Credibility shapes how regulators respond after serious incidents. It influences whether workers trust the investigation process. It determines whether leadership teams view safety professionals as strategic advisors or merely compliance administrators. One of the strongest ways to build that credibility is through mastery of incident investigation and organisatio

The Hidden Cost of Poor Investigations: What Businesses Miss After an Incident

When a workplace incident occurs, most organisations respond quickly. The scene is secured. People are interviewed. A report is written. Corrective actions are assigned. Operations resume. On the surface, the organisation appears to have managed the event appropriately. But in many businesses, the real damage begins after the investigation is closed. Poor investigations create hidden costs that compound over time. These costs rarely appear immediately on a balance sheet, yet they influence operation
